5 easy steps to creating your Journal

1. Download and Complete the Templates either online or on paper.

2. Use the first page of the Journal.pdf for your outside cover.

3. Print & Place completed Templates in Accompanying Sections in a Journal.

4. Place Journal in visible and accessible location for First Responders, Caregivers, & Family.

5. Print Additional Templates for car and travel purposes (optional).
