emergency. It provides a comprehensive overview of an individual’s current and past medical condition, medications, allergies as well as medical history, treatment preferences, DNR (Do Not Resuscitate) and Power of Attorney (POA).
In an instant, An individual may be incapacitated or unable to respond to critical medical questions. Individuals with memory or cognitive impairment may be unable to respond to critical medical questions. Care Companions or Care Givers may be unable to respond to critical medical questions. The Emergency Medical Journal outlines an immediate, organized and concise medical profile on behalf of the distressed individual. Informative templates are provided in an easy to complete format either on paper or online. Once completed the pages can be printed and/or downloaded to place in the Journal. The Journal can be printed for home, car, or travel purposes.
